WHAT IS HOME HEALTH CARE?


Country Manor’s Home Health Care is a medicare certified agency, and is offered to tenants residing on Country Manor Campus, and in the surrounding communities (up to 30 miles). 

Home Health Care is in-home clinical medical care provided by a Registered Nurse, Occupational Therapist, Physical Therapist or other skilled medical professionals. 

Home Health Care is often prescribed as part of a care plan following a hospitalization, illness, injury or chronic illness.

Home Health Aides and Home Makers are also available through Country Manor's Home Health Care. 

SERVICES


Home Health Care services include but are not limited to:
  • Skilled nursing services
  • Physical, Occupational and Speech Therapy
  • Administration of medications
  • Safety and health assessments
  • Monitoring serious illness and unstable health status
  • Wound care
  • Patient and caregiver education 
  • Nutrition therapy 
  • Injections and lab draws
Home Health Aide/Home Maker services include but are not limited to: 
  • Personal cares and grooming 
  • Meal planning and preparation
  • Basic housekeeping 
  • Laundry

Country Manor’s Home Health Care team works in coordination with Country Manor’s rehabilitation team (Champion Rehab) to bring unique services into the homes of clients. 
Country Manor’s Home Health Care is part of a larger organization that offers a complete continuum of care. Clients can rest assured that there is always help available, no matter what the circumstance.
