The Home Health Quality Improvement (HHQI) National Campaign presented Country Manor Home Health Care with a Milestone Two accomplishment on the Home Health Cardiovascular Data Registry. This achievement was based on the agency’s process for collecting and submitting patient data to the HHCDR.
