Country Manor’s Home Health Care and Assisted Living services are offered on Country Manor Campus and in the surrounding communities.
WHAT IS THE DIFFERENCE BETWEEN HOME HEALTH CARE AND ASSISTED LIVING?
HOME HEALTH CARE
Home Health Care is in-home clinical medical care offered to individuals in the community (up to 30 mile radius), and to tenants residing on Country Manor Campus.
Services are provided by a Registered Nurse, Occupational Therapist, Physical Therapist or other skilled medical professionals. Home Health Care is often prescribed as part of a care plan following a hospitalization, illness, injury or chronic disease.
Home Health Care may also include Home Makers.
Home Health Care services include but are not limited to:
- Skilled nursing services
- Physical, Occupational and Speech Therapy
- Administration of medications, including injections
- Safety and health assessments
- Monitoring of health status
- Wound care
- Cares associated with chronic illness
- Personal cares and grooming (Home Health Aide)
Home Maker services include but are not limited to:
- Meal planning and preparation
- Basic housekeeping
- Laundry
ASSISTED LIVING
Assisted Living is a supportive service offered specifically to residents residing on Country Manor Campus, in the privacy of their own apartment homes.
Services are provided by trained health care professionals and vary greatly based on circumstance. Activities often associated with Assisted Living services include dressing, eating, mobility, hygiene, bathing, meal preparation and shopping.
Assisted Living services include but are not limited to:
- Meal planning and preparation
- Basic housekeeping
- Laundry
- Health and exercise programs
- Social programs
- Personal cares
- Medication administration
- Skilled nursing services available as needed

UNIQUE TO COUNTRY MANOR
Country Manor’s Assisted Living Services are offered to tenants in the privacy of their own homes. We do not require clients to move as their health fluctuates.
Why should I consider Home Health Care and/or Assisted Living?
- Following an inpatient hospitalization, rehabilitation, or stay at a skilled nursing facility to help clients transition back to an independent lifestyle
- After a medication change in order to monitor for side effects and interactions
- When experiencing a decline in cognitive or physical abilities to help regain independence through therapy or learn new skills to adapt
- When assistance is needed to complete daily living tasks like grooming and meal preparation
- For individuals who live alone and may be at risk for social isolation
How does Home Health Care and Assisted Living work?
How do I pay for Home Health Care and Assisted Living?
- Covered by Medicare when prescribed by a physician and/or meet skilled criteria
- Medicaid for qualified individuals
- Private pay
